After being betrayed by one of his own tenants, Jamie is captured by government dragoons and imprisoned at Wentworth Prison, a fugitive facing execution. Claire and the MacKenzie clansmen mount a rescue attempt that initially fails, and Claire herself is captured by Captain Jack Randall, who threatens to rape her. Jamie offers himself in her place, and Randall releases Claire into the woods. Before leaving, Claire tells Randall the exact date of his death, which she knows from Frank's family genealogy. Sir Marcus MacRannoch then helps orchestrate a real rescue: his men distract Wentworth's guards while the clansmen drive a herd of cattle through the prison's underground passages, trampling a guard in the chaos, and pull Jamie out. He has been savagely beaten and sexually assaulted by Randall and is left physically and psychologically shattered.

Claire and the men bring Jamie to MacRannoch's stronghold, where Claire tends his wounds, though his spirit is nearly broken and he shows little will to live. As soon as he can travel, Jamie, Claire, and Jamie's godfather Murtagh flee across the sea to France, seeking refuge at the Abbey of Sainte Anne de Beaupré, where one of Jamie's uncles serves as abbot. There, in the sanctuary of the monastery, Jamie slowly begins to recover from his trauma with the help of the monks and Claire's steady presence, gradually reclaiming his sense of self and his bond with her.

Earlier in the story, Claire had already made her defining choice: after confessing to Jamie that she is a time traveler from 1945, he took her back to the stone circle at Craigh na Dun and gave her the chance to return to her own century and to Frank. She chose instead to stay with Jamie in 1743, binding her fate to his. That choice is reaffirmed by everything that follows, including the ordeal at Wentworth and the flight to France.

The novel closes on a quiet, intimate note at the abbey. As Jamie and Claire emerge together from a hot spring beneath the monastery, having survived the prison, the escape, and Jamie's slow healing, Claire tells him she is pregnant. Claire Randall, a former WWII combat nurse from 1945, accidentally traveled back in time to 1743 Scotland after touching a standing stone at the circle of Craigh na Dun. There she was taken in by the MacKenzie clan and met Jamie Fraser, a young outlawed Highland warrior with a price on his head from the English Crown. To protect her from the sadistic English officer Captain Jack Randall (the ancestor of Claire's 20th-century husband, Frank), clan leader Dougal MacKenzie arranged a marriage between Claire and Jamie, making her legally Scottish. What began as a marriage of necessity grew into real love, and Claire ultimately chose, when given the explicit option to return through the stones to Frank, to stay in the past with Jamie.

Along the way, Claire served as the castle's healer at Castle Leoch and befriended Geillis Duncan, another woman with unusual knowledge of herbs and medicine. The two were eventually accused of witchcraft; during the ordeal, Claire discovered that Geillis was also a time traveler from the future and was secretly pregnant by Dougal, and was involved in funding the Jacobite cause. Jamie and his men helped Claire escape the witch trial, but Geillis was left behind, and her ultimate fate was not resolved on the page.

Jamie took Claire to his family home, Lallybroch, where she met his sister Jenny and Jenny's husband Ian, and Jamie briefly reclaimed his role as Laird despite still being a wanted man. A tenant's betrayal led to Jamie's capture and imprisonment at Wentworth Prison, where Captain Randall tortured and sexually assaulted him after Jamie offered himself in exchange for Claire's safety. Claire and the clansmen, aided by Sir Marcus MacRannoch, staged a dramatic rescue using a cattle stampede as a diversion, pulling a badly traumatized Jamie out of the prison.
